HOYF Emergency Action Plan (EAP) and Injury Report - HOYF activates an Emergency Action Plan (EAP) if a HOYF player or volunteer is injured during a practice, game or other HOYF event. As soon as possible, a HOYF Injury Report should be completed by a HOYF Board Member, Coach, Assistant Coach, or Team Manager, and promptly turned in to the HOYF Commissioner or Deputy Commissioner.
